Prime Prescribers is a HIPAA-covered health care practice. This page explains what this website collects, and how your protected health information is used and safeguarded.

Part 1 — Website Privacy Policy
What this website collects
This website collects no personal information. There are no contact forms, no account creation, no newsletter sign-up, and no health questions anywhere on it. We do not run advertising trackers or third-party analytics, and we do not sell or share information about visitors, because we do not collect any.
Fonts and images are served from this website itself rather than from a third-party network, so browsing these pages does not report your visit to another company.
Links to our patient portal
Every button that says Become a Patient or Patient Login leaves this website and opens our patient portal, which is operated by our electronic health record provider under a HIPAA business associate agreement. Anything you enter there — registration details, your questionnaire, messages, results — is governed by Part 2 below and by the portal provider’s own terms.
If you email us at maryann@primeprescribers.com, we receive whatever you choose to send. Ordinary email is not a secure channel for health information, so please keep medical details out of it and use the patient portal instead. We use email addresses only to answer the question asked.
Children
This practice serves adults 18 and over. We do not knowingly collect information from anyone under 18.
Changes
If this policy changes, the revised version will be posted here with a new effective date.
Part 2 — HIPAA Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our commitment
Prime Prescribers is required by law to maintain the privacy of your protected health information (PHI), to give you notice of our legal duties and privacy practices regarding your PHI, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of this notice while it is in effect.
How we may use and disclose your health information
- For treatment. To provide, coordinate and manage your care — for example, ordering laboratory testing or imaging, sending prescriptions to your pharmacy, or sharing relevant history with another clinician involved in your care.
- For payment. To bill and collect payment for services, including providing a superbill you may submit to your own insurer.
- For health care operations. To run the practice, review the quality of care, and work with our business associates, each of whom is bound by a written agreement to protect your information.
- As required by law. Including public health reporting, reporting suspected abuse or neglect, responding to lawful subpoenas or court orders, health oversight activities, and to avert a serious and imminent threat to health or safety.
Uses and disclosures not described in this notice will be made only with your written authorization. We will never sell your PHI or use it for marketing without your written authorization, and you may revoke an authorization in writing at any time.
Your rights regarding your health information
- Access. You may inspect and obtain a copy of your medical record, including an electronic copy.
- Amendment. You may ask us to correct information you believe is incorrect or incomplete.
- Accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.
- Restrictions. You may ask us to limit how we use or disclose your PHI. We are not required to agree, except that we must agree to withhold information from a health plan for a service you paid for in full out of pocket.
- Confidential communications. You may ask us to contact you in a specific way or at a specific address.
- Paper copy. You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.

How your information is safeguarded
All clinical information is held in a HIPAA-compliant electronic health record and patient portal, accessed over encrypted connections, and available only to those who need it to provide or administer your care. Virtual visits take place over a HIPAA-compliant platform. We maintain business associate agreements with the vendors who handle PHI on our behalf.
Complaints
If you believe your privacy rights have been violated, you may contact us at maryann@primeprescribers.com, or file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints. You will not be retaliated against for filing a complaint.
Changes to this notice
We may change this notice and make the new terms effective for all PHI we maintain. If we make a material change, the revised notice will be posted on this page with a new effective date.

The patient portal is not monitored around the clock and is not for urgent or emergency care. Messages are typically answered within about one business day. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department.
